Overview

MyWPH is a secure patient portal that allows you to access certain data gathered during your stay in the Hospital. It is a free online tool to help you manage your care. MyWPH helps you keep a log of your medical history and share the information with others, if you choose to do so.

What information is available?

With MyWPH you can access your important health information, monitor chronic conditions, track your progress, share data with your doctors, and have the information readily accessible in the event of a medical emergency.

Examples of information provided are:

  • Demographic data
  • Medical problems
  • Vitals (blood pressure, breathing rate, heart rate, temperature, etc.)
  • Medications administered or prescribed
  • Final reports
  • Appointments

The information in MyWPH includes data from past patient visits to the hospital.

MyWPH does not contain your complete medical record. To obtain a copy of your complete medical record, call 914-681-2181.

How to sign up for MyWPH

After you are admitted to the Hospital and are settled in your room, a member of the Hospital staff will visit you to give you a special code to use in registering.

If you would like to register while you are in the Hospital, someone will help you with the process. If you prefer to sign up for MyWPH after you leave the Hospital, you can use the code that was given to you as long as you register within 30 days of issuance. If a longer time than that has passed, please call 914-681-1105 to request a new code.

Register and access the Patient Portal

If you need assistance in registering for MyWPH, please call 914-681-2503.

If a longer time then that has passed, to gain access to the Patient Portal, please print and complete the Release of Information Form and return to the White Plains Hospital HIM Department as instructed below.

  • By Mailing or faxing your request
    • Mail to: White Plains Hospital, HIM Department, 41 East Post Road, White Plains NY 10601
    • Fax: 914-681-2908 or 914-681-2905

Please include on the form that you are requesting a Patient Portal Code and indicate your email, fax or address in order to receive your code.

Once you receive the code, you will be able to sign into the Patient Portal to review or print your records. (There are no fees associated with the Patient Portal.)

Medical Records/HIM

The Medical Records Department, also known as the Health Information Management (HIM) Department, keeps documentation of each patient’s medical care during hospitalization. The medical record includes test results, medications, treatments or therapies, x-rays, and reports.

You can obtain your records in multiple ways:

  • By visiting the HIM Department at the hospital, Monday – Friday, 9 am – 5 pm
  • By obtaining access to the Patient Portal. To gain access to the Patient Portal, please print and complete the Release of Information Form and return to the White Plains Hospital HIM Department as instructed below. Please include on the form that you are requesting a Patient Portal Code and indicate your email, fax or address in order to receive your code. Once you receive the code, you will be able to sign into the Patient Portal to review or print your records. (There are no fees associated with the Patient Portal.)
  • By Mailing or faxing your request

To request your medical record in person or by mail/fax, a copy of a photo ID is required, and you will need to fill out a “Release of Information Authorization” form. Below are links to the forms in both English and Spanish:

All requests will be processed within 10 business days. Urgent requests will be handled with priority. Please note there may be a fee associated with your request.
