In preparation, below please find relevant information on when you may be eligible, how we plan to schedule patients and what to expect on the day of your appointment.
Eligibility
Currently in New York individuals in Group 1A and 1B are eligible to receive the COVID-19 vaccination. According to the New York State Department of Health, as of February 15, 2021 patients with specific documented medical conditions may also be eligible to receive the COVID-19 vaccine. To find out if you are eligible, visit the New York State Vaccination website.
New Yorkers with underlying medical conditions are required to attest on the New York State Vaccination website that they have a comorbidity. This form will be automatically generated during the online registration process and must be completed before you can schedule an appointment. NYS DOH also requires all individuals to complete the New York State COVID-19 Vaccine Form prior to their vaccination appointment. Proof of completion must be presented at the vaccination site on the day of your appointment. Should further evidence of these conditions be required, patients of White Plains Hospital, White Plains Hospital Physician Associates and Scarsdale Medical Group can most quickly and conveniently obtain a list of your underlying conditions through your MyChart account. To obtain these records:
- Log in to MyChart and go to the “Health” icon in the top navigation bar
- Under the “What’s in My Record?” dropdown, click either “Current Health Issues” OR “Medical History” to view your medical conditions
- Take a screenshot of the page with your name and information; or if you are connected to a printer, click the “Print” icon in the top right corner to print your record
Those who are not currently patients must provide a doctor's letter or medical information evidencing comorbidity.
Sign Up & Scheduling
When available, existing patients of White Plains Hospital, White Plains Hospital Physician Associates, White Plains Hospital Family Health Center and Scarsdale Medical Group, will schedule their vaccination through their MyChart account. If you have not activated your MyChart account, please register now by clicking the link below. We will update our patients via MyChart and text messaging once appointment scheduling becomes available.
