Senior Services at White Plains Hospital

Senior Services at White Plains HospitalThe Myra and Louis Salzer Senior Services Program at White Plains Hospital is dedicated to improving the quality of life for older adults who have chronic health problems. The program provides assistance so that patients may stay at home and avoid unnecessary admissions to nursing homes, hospitals and other facilities. The program serves residents of central Westchester who are age 60 and older.

The Senior Service Program includes:

Outpatient Services

A gerontological nurse provides a free initial consultation where health problems and needs are assessed. The experienced staff then compiles the most suitable community resources to meet the patient's needs. Members of the team are available for ongoing services. Consultation with your private physician continues throughout the process.

Telephone Reassurance

To monitor the progress of our patients and for their peace of mind, we maintain regular telephone contact with our active patients. As a result, we often detect problems at an early stage, and assist in their resolution.

Voice Care

Voice Care is a two-way emergency response device installed in a patient's home that offers a direct link to help in case of an emergency, twenty-four hours a day.

Nursing Home Placement

We assist patients and their families in every phase of placement to a nursing home facility. Our knowledgeable staff provides information about facilities and the process and guides the family through the application and placement procedure. We provide escorted visits to the nursing facility and support during the actual transition. We also follow-up with a post-placement visit to the facility.

Mall WalkersMall Walkers
