Your treatment team will work with you and your doctor toward a satisfactory discharge from the hospital. You will receive your written discharge plan, which includes information for your care at home, shortly before you leave. We will be happy to explain any aspect of your treatment. Please sign your plan and return it to the nurse or discharge planner who gave it to you. If you have any questions, ask your nurse or call the Utilization Review Department at extension 1143.
Before you leave, your nurse will make sure that you have been given any instructions and written prescriptions that you will need.
YYour doctor will let you know when you will be discharged. Please make arrangements for transportation as early as possible. Check out time is 9 am. If you need a taxicab or other transportation, please ask the clerk on your nursing unit to help you. Before you can be discharged you or a member of your family must check out with the Cashier's Office regarding your account. American Express, Visa and MasterCard are accepted by the Hospital.
If someone is picking you up, here are a few things you should tell them:
- Plan to come to the Hospital about one hour before you are discharged.
- After taking care of your accounts, your driver will be given a "Patient Clearance Slip" and directed to your floor: When your driver arrives on your floor, he or she should give the "Patient Clearance Slip" to the clerk on the nursing unit. The clerk will then arrange to have you transported by wheelchair to the main lobby.
If you do not have a relative or friend to escort you home, arrangements will be made to have you transported by wheelchair to the cashier's office for check out. Please be sure all of your personal belongings are with you as you leave.
About Your Bill and Insurance
Insurance plans such as Empire Blue Cross, Medicare, commercial insurance and Medicaid, will cover most of the costs of your hospitalization after your deductible is paid. You will be responsible for paying any deductible and charges not covered by your insurance as well as any television and telephone charges. If you have questions about your bill or your insurance coverage during your stay, contact the Credit Office at 681-1016.
Any questions concerning your bill should be directed to the Patient Accounts Department. This office is open from 8 am to 4 pm. Telephone inquires are welcome from 10 am until 3 pm.

Notice of Availability of Charity Care
In recognition that all patients who seek health care from White Plains Hospital may not have the financial resources or insurance coverage to afford care, the Hospital will make available a reasonable amount of uncompensated services to persons meeting the eligibility requirements.
Eligibility for free care will be limited to persons whose family income is not more than the current poverty guidelines established by the Department of Health and Human Services. Persons whose income is greater than the guidelines are eligible for hospital charity care at a reduced-fee basis. For more information call the Credit Department at
The Hospital does not issue birth certificates but it does send information about births to the City of White Plains which is the legal entity that issues birth certificates to those born at the Hospital. If you need a copy of a birth certificate, you have to request it from the City of White Plains. For your safety and the safety of others, if you need to use medical supplies after your discharge, WPH collects used hypodermic needles (sharps) for safe disposal. Used sharps may be brought to the Security Office, located in the Emergency Room waiting area, daily between 8:00 am and 4:00 pm.
